    • The services are intended to assist in managing your appointments, payments, and communication with your healthcare provider. You agree to use the services for their intended purpose and in accordance with all applicable laws and regulations.
  4. User Obligations
    • You are responsible for providing accurate and complete information when using our services. False or misleading information may result in the termination of your access to the services.
  5. Intellectual Property
    • All content and materials provided by Branda, including but not limited to text, graphics, logos, and software, are the property of Branda or its licensors and are protected by intellectual property laws. You agree not to reproduce, distribute, or create derivative works from any content without prior written permission.
  6. Limitation of Liability
    • To the maximum extent permitted by law, Branda and its affiliates shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising out of your use of or inability to use the services.
  7. Indemnification
    • You agree to indemnify and hold Branda and its affiliates harmless from any claims, damages, losses, liabilities, and expenses (including legal fees) arising out of your use of the services or your violation of these Terms and Conditions.
  8. Governing Law
    • These Terms and Conditions are governed by and construed in accordance with the laws of the State of Florida.

Cancellation Policy

One of Branda’s services is to facilitate the booking of your dental appointments. We recognize that schedules can change, leading to the need for appointment cancellations or rescheduling. Please note that Branda does not set or enforce cancellation policies for our dental service providers. We advise contacting the dental office directly for details on their specific cancellation policy.

Dispute Resolution

Our commitment at Branda is to ensure a satisfactory experience for all our users. Should you face any issues related to the services received or payment, we recommend contacting the dental service provider directly as the first course of action. If further assistance is needed or if the issue remains unresolved, please reach out to Branda Customer Support at support@brandacare.com. Our team is dedicated to assisting you towards a fair and timely resolution.

Online Patient Consent for PHI

Introduction: This document serves as your consent and authorization for Branda to facilitate the collection, use, and disclosure of your Protected Health Information (PHI) in compliance with applicable healthcare laws and regulations. Please note that Branda acts solely as a facilitator and not as a primary handler of your PHI.

Patient Consent: By submitting this form, you authorize Branda to assist your healthcare provider in handling your PHI for treatment, payment, healthcare operations, and other lawful uses. Your healthcare provider remains the primary entity responsible for your PHI.

Purpose of Use and Disclosure: Your PHI may be utilized for enhancing treatment processes, processing payments, administrative operations, and other purposes permitted by law. Branda acts solely to support your healthcare provider in these activities.

Information to be Disclosed: This includes medical records, goods and services rendered, treatment and diagnostic information, billing and accounting information, and any other health-related information provided by your healthcare provider.

Recipients of Your Information: PHI might be shared with medical practitioners, insurance entities, or legally authorized third parties by your healthcare provider, with Branda assisting in this process.

Security Measures: Branda employs stringent security protocols, including encryption and secure storage, to protect your PHI against unauthorized access, alteration, or destruction. However, the primary responsibility for PHI security rests with your healthcare provider. See Privacy Policy legal for more information.

Patient Rights:
You have the right to:

  • Inspect and obtain a copy of your PHI.
  • Request amendments to your PHI.
  • Limit certain uses or disclosures.
  • Obtain a record of disclosures of your PHI.
  • Revoke this consent in writing, subject to limitations.

Please note: These rights should be exercised by contacting your healthcare provider directly, as they are the primary handlers of your PHI. Branda acts solely as a facilitator in this process.

Communication of Breaches: In the unlikely event of a breach involving your PHI, you will be promptly informed as per regulatory guidelines. Your healthcare provider is primarily responsible for such notifications.

Duration and Revocation: This authorization remains valid until revoked. You may withdraw your consent at any time by sending a revocation request to privacy@brandacare.com. Please understand that actions taken before revocation will not be affected.
